                               Enrollment/Transition Process for Students Entering Durham Public Schools from an Alternative Placement

      Definitions

·           New Student – a student who has never attended DPS.

·         A Re-entering Student – a student who last attended DPS, but has been withdrawn by the parent of the school.

·         Base School Transition Team – A multidisciplinary team consisting some or all of the following members: school counselor, social worker, psychologist, teacher(s), EC Facilitator, parent or legal guardian or          others deemed necessary to provide supports for the student’s success in his/her school environment.

Psychiatric Hospital/Residential Setting

·         The Office of Student Assignment will contact the counseling office (secondary schools), or main office (elementary schools) of the respective base school to inform them of the newly assigned student, and       fax the completed assignment packet to the contact person.

·         The base school counselor will compile student data and conduct a transition team meeting to ensure supports are in place for the student.  If the student has an IEP, the EC Facilitator will be a part of               the transition team.

·         The base school transition team may refer the student to Lakeview School after attempts to serve the student at the base school have proven unsuccessful. The base school transition team may conduct a            transition meeting with the Lakeview School transition team.

EC/Self Contained Setting

·         The Office of Student Assignment will fax completed student information, student assignment and IEP (if available) to the EC Office.

·         The EC Office will make the school assignment and send the assignment to the respective EC School Facilitator and/or EC Coordinator.

·         The EC Facilitator and/or EC Coordinator will notify the parent of the school assignment and arrange an IEP transition meeting for the student at the assigned school.

      Prison, Jail, Youth Detention Center

·         The Office of Student Assignment will investigate the student’s status from the facility. If the student was sentenced for a minor offense, such as breaking probation, etc. he/she will be assigned to the base       school. If the offense is more than minor, Student Assignment will fax the student assignment packet to the Lakeview School Registrar.

·         The transition team and/or the EC Facilitator will compile student data and assign the student’s course schedule.

·         The Lakeview School transition team will conduct a transition meeting with the base school to set a date for the student to begin at his/her base school. 

            Alternative School

·         The Office of Student Assignment will investigate the student’s status from the sending school.  If the student has completed his/her assignment from the alternative school placement, the student will be            assigned to his/her base school.  If the student has not completed the assignment from the alternative placement, he/she will be assigned to Lakeview School.

·         The base school transition team may refer the student to Lakeview School after attempts to serve the student at the base school have proven unsuccessful.

     Charter, Private or Home School

·         The Office of Student Assignment will investigate the student’s status from the sending school. If the student was suspended for an offense that DPS would suspend for, DPS will honor the suspension. If the            offense is one that DPS would not suspend for, the student will be assigned to his/her base school.

·         If the student is classified EC/IEP and the suspension is a suspendable offense according to DPS policies, the student will be assigned to Lakeview School (secondary), base school (elementary)

·         The Lakeview School transition team will conduct a transition meeting with the base school prior to the end of the student’s suspension to set a date for the student to begin at his/her base school.
